Unmodified (aside from the McPherson pick holder) and all original to my knowledge. I've never had the original case, so it's currently in a relatively new Epiphone case that will be included.
I took it to Noll Guitars last summer to have it looked at, and they said the knobs are a bit scratchy, but otherwise all it needs is a setup, but I didn't have it set up because I figured the buyer might want to put different strings on it. (Right now it has La Bella Deep Talkin' flatwounds, and the action is way too high.) I should also note that the screws on the bridge seem stiff to me even when the strings are detuned, so that may need professional attention as well.
I previously had this listed as a 1963, but I was recently informed that serial number guides put this serial number in 1965. So I checked the potentiometer codes, and they indicate that they're from the 21st week of 1964 (see photo). So I'm going with 1964 for the date based on the pot codes. But please keep in mind that these basses were sometimes built over a period of time, so dating them is not an exact science! The features of the body identify it as being from 1962 through 1965.
See photos for the cosmetic condition. It's been played a lot over the decades, so the paint has come off of some metal parts, and there's significant rust and metal corrosion, especially on the bridge (I believe the spots there are corrosion as opposed to mildew as they don't come off with cleaning). There's some discoloration on the upper part of the neck; I don't know if that's a water stain, a flaw in the wood, or what. I find the aged appearance to be quite appealing, particularly the yellowing on the nitrocellulose on the back of the neck. It's fun to play (or at least it will be after it gets set up properly again), but I don't personally have too much use for the EB-3 sound in the music I record, so I'd rather let this go to someone who can really appreciate it!
