Our rapidly expanding Community Based Service Programs are in need of a number of reliable, flexible, self-motivated team members to join our dynamic community.
Intensive Home-Based Clinician (Fee-for-service) will work within ourEnhanced Outpatient Services (EOS). EOS is a home/community-based program. The EOS Team consists of a Clinician and Care Coordinator who work together to provide home, school and/or community based services for children/teens/adults and their families who require intensive support for successful functioning in the community. This program is intended to prevent hospitalization or higher level of care, or to support successful transition from a higher level of care back to the community. EOS is only available to children/families/individuals covered by Neighborhood Health Plan/RI and United Health Care/RI. Some of the duties and responsibilities of this position include assessment and treatment planning; crisis intervention; maintaining case records; collaborate with outside service providers; transportation assistance; care coordination; and client advocacy as needed. Competitive compensation and mileage reimbursement offered. Schedule is flexible.
Qualifications of Clinicians: Master of Social Work Degree or equivalent Counseling Degree required. Rhode Island Licensure preferred (LICSW, LCSW or LMHC). Experience working with children and families in community-based settings preferred. Experience in the treatment of trauma preferred. Bilingual candidates (English/Spanish) encouraged to apply.
