🕒 Part-Time | Monday–Friday | Day Shift
💵 Pay: $22–$25 per hour
Joyful Living Adult Day is part of the Sevita family, a mission-driven organization committed to providing high-quality services and meaningful support to the individuals we serve. Every team member plays an important role in helping participants live fulfilling, community-integrated lives.
The Case Coordinator provides organizational and programmatic support, including admissions assistance, direct participant support, care coordination, and communication with families, staff, and external partners.
Support and coordinate services to ensure participant needs are met.
Assist participants with understanding their goals and programs.
Serve as a liaison for families, providing updates and resources.
Organize and lead participant conferences.
Review evaluations, progress notes, and discharge summaries.
Maintain accurate participant documentation.
Assist with discharge planning and follow-up.
Communicate case-specific updates with team members.
Attend required meetings, including clinical meetings.
Review monthly bills and report discrepancies.
Coordinate participant incentive accounts with the Case Manager.
Perform other duties as assigned.
🎓 Bachelor’s degree in a Social Services–related field required
🧑⚕️ Two years of experience supporting individuals with disabilities in a medical or rehabilitation setting
